In regular competitions, Bashkimi was the leader with 20 wins and eight losses, while Trepça was second with 19 wins and nine losses.
These two teams faced each other four times this season in the Super League, where they had two victories each, with all victories being at home.
They also faced each other in the Cup final, where the 'Miners' triumphed in the capital.
Bashkimi is seeking its second title in history, its first after 2018, while Trepça is the current champion, having won this trophy five times in its history.
The first match will take place on Sunday in Prizren, while the team that scores four wins wins the champion title.
